Important School Vaccine Information for Middle School and High School Parents!
We are excited to announce a partnership with the Putnam County Health Department to provide REQUIRED school vaccines for students in grades 6-12. This initiative is scheduled for Wednesday, March 20, 2024, providing a convenient opportunity to ensure your child is up-to-date with essential vaccinations.
Vaccines Offered and Requirements:
Required Vaccines:

Tdap (Tetanus, Diptheria, and Pertussis):
- Required after age 10 and before the start of 8th grade.

Meningococcal (Meningitis):
- First dose required after age 10 and before the start of 8th grade.
- Second dose required after age 16 and before the start of 12th grade.
Recommended but not Required:

Meningococcal B:
- Recommended for 11th-grade students.

HPV (Human Papillomavirus):
- Recommended for students ages 9 and older.
How to Participate:
This vaccination opportunity is not only a requirement but also a chance to safeguard your child's health before the summer break. If you wish your student to receive these immunizations at school, please complete the consent form linked here by Friday, March 15th.
Exemption Information:
If you do not wish your student(s) to receive the required immunizations, a Religious or Medical Exemption card must be on file with the school before the 1st day of school in August. This is mandatory for students entering 8th and 12th grades. All students entering 8th or 12th grade must have the required vaccines or an Exempt Form on file before the 1st day of school in August.
