This page is intended for new hires who are being rehired into the district but have not had a separation of more than a year.

- The Human Resources Department is unable to give tax advice regarding the W-4 form. If you have questions about your withholding please consult with your accountant prior to your processing appointment.

- Information regarding the Oregon State W-4 can be found on the Oregon Department of Revenue website by clicking here.
- Child Abuse & Sexual Conduct Disclosure Release
- This is only required if you have worked for another education provider since you last worked for the Eugene School District 4J. If you did not begin employment with another education provider after you separated from employment with 4J then you do not need to complete this form.

- PERS Notice of Employment
- Make a copy of your COVID-19 vaccination card and bring it to orientation
- Per Gov. Kate Brown’s mandate all public school district staff and volunteers must be fully vaccinated against COVID-19. “Fully vaccinated” is defined as 14 days after receiving all required doses of an approved COVID-19 vaccine (currently two shots of Pfizer or Moderna vaccine, or one shot of Johnson & Johnson vaccine). All staff members will need to provide proof of COVID-19 vaccination. Exemptions may be made only for eligible medical or religious reasons. There will not be an option to opt out for other reasons or choose testing in lieu of vaccination, under the state mandate.

- You will not be able to activate your 4J email until you have completed processing and are reactivated in the HR/Payroll system. Once you receive notice that you are active in the system again then you will be able to activate your 4J email account.
