Unfortunately, we don’t have the ability to extend the process appointment for those that don’t complete the required documents. Due to time constraints if you do not complete the required paperwork prior to arriving for your scheduled new hire orientation you will be asked to attend the next available processing time.

- To be completed before orientation:
- Take your employee ID badge photo
- Follow the process and guidelines provided by clicking here. If you are not able to take a badge photo that meets the provided guidelines prior to orientation then we will take one for you at orientation.
- Make a copy of your COVID-19 vaccination card and bring it to orientation
- Per Gov. Kate Brown’s mandate all public school district staff and volunteers must be fully vaccinated against COVID-19. “Fully vaccinated” is defined as 14 days after receiving all required doses of an approved COVID-19 vaccine (currently two shots of Pfizer or Moderna vaccine, or one shot of Johnson & Johnson vaccine). All staff members will need to provide proof of COVID-19 vaccination. Exemptions may be made only for eligible medical or religious reasons. There will not be an option to opt out for other reasons or choose testing in lieu of vaccination, under the state mandate.

- Until you are entered into the system you will not have an ID number. Do not worry about filling out that one line on this form.
- Federal W-4
- The Human Resources Department is unable to give tax advice regarding the W-4 form. If you have questions about your withholding please consult with your accountant prior to your processing appointment.
- Oregon State W-4
- Information regarding the Oregon State W-4 can be found on the Oregon Department of Revenue website by clicking here.

- Note that this form is required for all new employees. For more details please read the instructions linked above.

- It is encouraged to submit official transcripts at the time of processing. This is not required for a Guest Teaching position but will help if you transition to a contact teaching position.
