Eugene School District 4J offers a vision plan as part of your MODA medical plan. It is included in your premium so there is no additional cost to you and no election to make once you make your medical plan choice. All of your dependents covered under your medical plan are also covered under your vision plan. You can see this chart for coverage details. You cannot waive this coverage if you enroll in a medical plan and you cannot choose to enroll in vision only coverage.
