The Wellness clinic has instituted protocols to keep patients and staff safe during the Covid19 pandemic.
- The lobby is closed and walk-ins are not accepted. Please call ahead for scheduling. Ring the bell when you arrive.
- Face masks/coverings are required.
- Patients are asked not to bring additional visitors. We ask only one parent per minor child be in attendance.
- Televisits (video chats) and telephone calls are available depending on preferences and reason for visit.
- Ill patients are often provided services through video, phone, and curb side.
- Mental Health services are available through video or off site (counseling is currently not being offered on site).
- Routine and preventative care is being offered.
- Flu shots available.
- Covid19 testing for contact or symptoms performed onsite. Rapid testing available in 15 min.
- Potential aerosolizing procedures of nebulizer medication delivery not available at this time.

Wellness Clinic General Information:
The Wellness Clinic is a medical clinic run through a joint effort of the District and its employees (via the Joint Benefit Committees) to provide insurance-eligible 4J employees and their dependents with pre-paid medical care for routine needs. 4J Retirees and their dependents who are enrolled in retiree health insurance through 4J are also eligible to use the clinic. *Reminder* 4J employees and retirees who enroll in 4J/OEBB Kaiser Health as their primary health insurance coverage are eligible to use the clinic. There is no out of pocket cost for visits to the clinic but your insurance will be billed.
Clinic services are provided by Cascade Health. Cascade Health is honored and excited to have the opportunity to work with you and your family through the 4J Wellness Clinic. Getting set up for your first 4J Wellness Clinic visit is easy. For new patients, please follow the three easy steps below:
- Call (541) 686-1427 to schedule your appointment.
- Have your health insurance information available when scheduling your appointment, even if you are not insured through the Eugene School District 4J’s insurance.
- You will need to fill out new patient paperwork for your first visit available electronically.
For existing patients:

Clinic Services
The clinic provides a full range of primary health care, diagnostic tests, minor surgery and preventative care. Annual physicals are available, as well as school, sports and camp physicals for children. See below for a listing of services.
Primary care services including chronic disease management and screening for ages 4 and older. Workplace injuries and workman’s Compensation visits are not seen at the clinic. Other minor injuries can be evaluated.

The 4J Wellness Clinic is recognized as a Patient-Centered Primary Care Home (PCPCH) by the Oregon Health Authority which honors our commitment to providing high quality, patient-centered care. Both nurse practitioners at the 4J Wellness Clinic are also primary care providers in the coordinated care Moda 360 Program that provides enhanced benefits and cost savings.
Michelle Davila, FNP, DNP
Michelle has worked at the 4J Wellness Clinic since 2018. She has previously provided primary care at two federally qualified health centers (FQHC), one of which was a school-based health center. She enjoys working with individuals to optimize health through nutrition and daily activity. Her interests include hiking, reading, gardening and crafts

Shannon Micheel, FNP-BC
Shannon has been serving patients at the 4J Wellness Clinic since 2019 after working as a Peace Corps Medical Officers in Madagascar. Prior to working overseas, Shannon worked for 10 years providing primary care at the Community Health Centers of Lane County. She is passionate about helping patients achieve wellbeing from a holistic approach. She is an avid cycler and runner and enjoys spending time in the outdoors with her family.
