Notice Requirements
Generally, employees must give 30-days’ advance notice of the need for leave. If it is not possible to give 30-days’ notice, an employee must notify their Principal/Supervisor and/or Human Resources as soon as possible and, generally, follow the district’s usual procedures.
Leave of Absence Request Forms
Medical Leave of Absence Packet = Leave to care for one’s own serious health condition.
Family Medical Leave of Absence Packet = Leave to care for an immediate family member who has a serious health condition.
Non-Medical Leave of Absence Packet = All leave types not pertaining to medical necessity. This includes bereavement leave, personal leave, part-time leave, professional leave, parental leave (to care for your newborn child, newly adopted child, or to provide foster care), and all other leaves allowed under the District/EEA Bargaining Agreement, the District/OSEA Bargaining Agreement, or the District/4JA Statement of Understanding.
Intermittent Leave Tracking Form = This is the form needed every month while on an intermittent leave. The time submitted on this form while on an intermittent leave documents the time away from work covered under your approved leave of absence.
Unpaid Days
Leave Request for Unpaid Days = Unpaid leave equal to or less than five (5) workdays in length.
- Note: if you are requesting a leave longer than five (5) workdays, you will need to complete one of the regular leave forms listed in the previous section above.
