Enrollment – New hires or newly eligible employees will enroll within the first two weeks of their new position. See the New Employee Tab for additional information. Existing employees can change their enrollments each year during open enrollment. There are a few qualifying life events that allow specific changes outside of open enrollment period. Please see the Life Events section for additional details.

Active Employee Rates — 2016-2017
While all benefit eligible employees are offered the same benefits, the cost of those benefits will vary depending on many factors. Below are charts where you will find rates based on your classification.

To find your rate, you will look for your FTE on the left column, then your appropriate level of coverage (Employee only, Employee + Spouse etc….) and then move right until you see the plan you would like. The top section include the MODA dental rate, the middle chart includes Willamette Dental and the bottom chart does not include dental.12 Check Employees: 10 Check Employees (some food services and transportation employees): There are a few that will only receive 10 checks throughout the year. If you are unsure, please refer to you employment offer letter or email from your HR Coordinator. |
Licensed | Connexus/PPO Synergy/CCM Licensed employees have a composite rate. This means you pay the same amount regardless of your level of coverage (Employee only, Employee + Spouse etc…) |

Licensed Substitutes |
Benefit eligible substitutes also have the opportunity to purchase medical, prescription, dental and vision insurance. There are two options for rates depending upon the individual eligibility. For additional information please contact the HR Office 541-790-7660. |

2016-2017 Benefit Enrollment/Change Forms
2016-2017 OEBB-Appeal Form – This form is required if you need to challenge a decision made by OEBB or missed an enrollment deadline. This form gets returned to OEBB directly, the addresses and numbers are on the form itself.
2016-17 Benefit Enrollment Essentials – Use this guide to find out basic information to help guide you through enrollment.
2016-17-New-Hire-Enrollment-Form – This form is for new hires and newly benefit eligible employees to complete. This enrolls them in medical, Rx, dental, and vision coverage. This form is also used to waive coverage as well as designate beneficiaries. Health and Medical Plans
2016-2017 – Mid-Year Change Form – This form is used for employees that are currently enrolled and need to make a change to their elections (must have a valid QSC) or their beneficiaries.
2016-17 Flex Spending Enrollment Form -This form is to enroll in the Flexible Spending Account plans for both Health Related Expenses as well as Dependent Care Expenses
Wellness Clinic Enrollment Form – This form is used by all benefit eligible employees for initial enrollment. You only need to complete this form if you are waiving other health benefits.
FSA Change Form (Pacific Source) – This form is for mid-year changes to Dependent Care FSA only. Please return completed form within 30 days of the change. Please note that the form needs to be returned to Human Resources by the 15th of the month for changes to current month pay cycle.
