Shared Housing
When a family is living in an informal housing arrangement, such as with a relative or friend, or when the parent/guardian of the student cannot provide the proof of address documents in their name as legally required to enroll their student, then a Shared Housing form is alternate way to provide proof of address for enrollment in a Eugene School District 4J school.
Required Proof of Age and Address

Online Shared Housing Form (English/Spanish) A parent/guardian can submit this form to the School Choice and Student Assignment Office. The information will be reviewed and, if verified, the attending schools will be notified.
If the information provided on the form is unable to be verified, and at the discretion of the Superintendent’s designee, signatures and proof of address for the parent/guardian and/or the homeowner/leaseholder, or a notarized Shared Housing Form will be required.

You can download and complete a Shared Housing Form. The parent/guardian and the homeowner/leaseholder signatures on the Shared Housing form must be notarized. Both parties must bring a current and valid picture identification to have their signature notarized. Free notary services are available through the Eugene School District 4J School Choice Office. Please email schoolchoice@4j.lane.edu for more information or to request a printed version of the form be mailed.
Completed original forms with attached proof of address must be mailed or delivered to School Choice and Student Assignment Office, Eugene School District 4J, 200 North Monroe Street, Eugene, OR 97402.
** Until further notice, the 4J Education Center is closed to visitors. Notary appointments will be scheduled upon re-opening. A mail slot at the front entrance is available to drop off completed and notarized Shared Housing forms .**

If the shared housing is a temporary living arrangement due to loss of housing or economic hardship, please contact the 4J McKinney-Vento office, 541-790-6888, for more information regarding eligibility for additional educational services under the McKinney-Vento Education Act.

Viviendas compartidas
Cuando una familia está viviendo en un arreglo informal de vivienda, como con un familiar o amigo, o cuando el padre o guardián del estudiante no puede proveer comprobante de domicilio con documentos con su nombre legal requeridos para matricular a su estudiante, entonces hay una manera alternativa para hacer este proceso de matrícula en el Distrito Escolar de Eugene 4J llenando el formulario de vivienda compartida.

Formulario de vivienda compartida (en inglés/español) El padre o guardián debe llenar el formulario para la escuela de su elección y la oficina de Colocación de Estudiantes. La información será revisada y si se comprueba, se notificará a la escuela de asistencia.
Si la información dada en el formulario no se puede verificar, a discreción del designado por la superintendente, se requerirán firmas y otro comprobante de domicilio para que el padre o guardián y/o para la persona en el contrato de arrendamiento adjunten o entreguen al distrito, y/o un formulario de vivienda compartida notarizado será requerido.
Imprima el formulario de vivienda compartida para ser notarizado
Usted puede imprimir para completar el formulario de vivienda compartida, varios lugares de hacer fotocopias ofrecen los servicios de notario a un precio módico. El padre o guardián y la persona en el contrato de arrendamiento deben firmar el formulario en presencia de un notario. Ambos deben llevar una identificación válida para que se pueda notarizar el formulario. La oficina del Distrito Escolar de Eugene 4J ofrece servicios de notarización gratuitos con cita. Por favor escríbanos a schoolchoice@4j.lane.edu para más información o para solicitar que se le envíe el formulario por correo.
El formulario lleno con el comprobante de domicilio engrapado puede ser enviado por correo o llevado a la Oficina de Elección de Escuela y Colocación de Estudiante en el Distrito Escolar de Eugene 4J en la 200 North Monroe Street, Eugene, OR 97402.
** Por el momento el Centro de Educación de 4J está cerrado al público. Las citas gratis para notario se programarán hasta su reapertura. La puerta principal tiene una ranura para la correspondencia donde puede echar su formulario completo y ya notarizado.**
