Virtual Charter Intent to Enroll Request Form / Formulario de intención para matrícula en una escuela charter virtual

Before enrolling a student in a virtual public charter school, the parent or guardian must submit an “intent to enroll” notice to the school district where the student resides. Oregon state law limits the number of students who can enroll in virtual charter schools not sponsored by the district, without the school district’s approval, to 3% of the district’s resident students.

Complete the following form separately for each student you wish to enroll in a virtual public charter school. If the virtual charter school provided you with an "Intent to Enroll" form, please upload it as an attachment. The district will respond within 14 days.

All fields are required except the attachment at the bottom.

Antes de matricular a un estudiante en una escuela pública charter virtual, el padre o guardián debe entregar el anuncio de “intención de matrícula” al distrito escolar donde el estudiante reside. Las leyes de Oregón limitan el número de estudiantes que pueden matricularse en las escuelas charter virtuales no patrocinadas por el distrito, sin la aprobación del distrito escolar, al 3% de los estudiantes residentes del distrito.
