4J Wellness Clinic News
The 4J Wellness Clinic is now certified by the state of Oregon as a Patient Centered Medical Home (PCMH). Additionally, the clinic staff is working with Moda to complete the steps necessary to become a Synergy Network PCMH.

The PCMH is a way of organizing primary care that emphasizes care coordination and communication leading to higher quality care and lower costs, and can improve patients’ and providers’ experience of care. In a PCMH model, a team of healthcare professionals work together with you and your family to help you achieve your health goals. Your health “team” is comprised of every health care professional that impacts your life – your primary care physician, pharmacists, therapist, other health care professionals, and most importantly, you! Through communication with your other health care providers, a PCMH strives to ensure safe, effective care. 

Synergy Network Medical Home Election
If you enrolled in a new Synergy Network plan during open enrollment, remember that you must choose a Moda Medical Home (MMH). If you are covering dependents on your plan, they too must choose a MMH. Each member of your family can choose a different MMH if they desire.

If you would like to change the MMH you elected during open enrollment or if you opted to contact Moda directly after open enrollment to name your MMH, you may do so by calling Moda Health customer service at 1-866-923-0409. You can also send an email to medical@modahealth.com.

Quarterly Cholesterol Screenings
The first drop-in cholesterol screening at the 4J Wellness Clinic will be held on Wednesday, October 1st.  

These screenings are done on a first come, first-served basis from 7:00 to 8:00 a.m. and are a FASTING cholesterol finger stick with results available in 5 minutes. FASTING means nothing to eat or drink but plain water for at least 10-12 hours. For the rest of the 2014-15 school year, the 7:00-8:00 a.m. drop-in cholesterol screening tests will be available on:

  • Wednesday, January 7, 2015
  • Wednesday, April 1, 2015
  • Wednesday, July 1, 2015

4J Flu Shot Clinics Fall 2014
Three flu shot clinics are scheduled for you to get this year’s vaccination.

Ed Center Auditorium, from 3:00 PM – 5:30 PM

  • Tuesday, October 7
  • Thursday, October 9

Transportation Dept., 9:00 AM – 11:00 AM

  • Tuesday, October 7

Dress accordingly. The nurses will need easy access to the upper arm.

Flu shot eligibility extends to:        

* all active employees (even those who are not insurance eligible due to less than half-time status)
* All insurance eligible dependents of active employees, including those who have waived coverage
*All retirees and their dependents that are currently covered on the 4J insurance
*Student teachers and substitutes (licensed and classified)

In this case, “dependents” refers to spouses and domestic partners, as well as insurance eligible children age 10 or over. Our particular vaccine is not approved for children under age 10–parents with children under 10 years of age should contact their pediatrician. All children 10-16 years of age must have a parent or guardian with them to be vaccinated.

Can’t make the flu shot clinics?
The following participating chain pharmacies provide vaccines: 

  • Fred Meyer Albertson Savon
  • Target             Rite Aid
  • Bi-Mart Safeway

Show your Moda ID card to the pharmacist for billing before receiving a vaccine-otherwise, it may not be covered.
Cascade Health Solutions offers drop-in flu shot clinics Mondays and Fridays starting September 22nd at the clinic offices located at 2650 Suzanne Way Ste 200. Please call 541-228-3100 for more information.

“Double-Match” 403(b) Retirement Savings Incentive Program
Don’t miss out! As part of the collective bargaining agreement between Oregon School Employees Association and the District, you have a unique savings opportunity. If you commit a small portion of your income into a retirement savings account, the district will not just match it; they will double-match it! The District will pay an amount equal to 1.25% of your monthly salary into a Tax-Sheltered 403(b) Savings account, if you commit to one-half (.625%) of that amount.

For administration purposes, the percentage “employee contribution with double district match” is limited to a choice of three (3) vendors.
To get started, contact the vendor of your choice to open an account.
Next, complete a District 4J Salary Reduction Agreement. Please make a copy for your records, and deliver the original to the Payroll department at the Education Center.

Drop-in and take this opportunity to learn how to “jump start” your retirement saving with a Tax Sheltered Annuity This event will include all 403 (b) TSA Vendors eligible to receive contributions.

Unable to attend the 403(b) TSA Vendor Fair?
Contact the vendors directly for information about investment options. Since there are fees charged by all 3 of the vendors, it is important to ask them to disclose all fees before you make a commitment.

If you need a copy of the District 4J Salary Reduction Agreement, check with your vendor, or pick one up in Financial Services.

Rates and Deductibles
The new Medical, vision, pharmacy and dental plan year begins October 1, 2014 and goes through September 30, 2015. The deductible and accruals toward maximum out-of-pocket reset October 1st, regardless of whether they were met the previous plan year. However, if you did not meet your deductible during the previous plan year and had continuous coverage, any expenses applied to your deductible during the last 3 months of the previous plan year will be carried over and applied to the deductible for the new plan year.
The new plan year insurance rates that take effect October 1 will be reflected in the October 31st paychecks. Please check your paycheck or bank statement to verify you have been charged the correct amount for your insurance elections.
