Rates and Deductibles 2016-17

The new medical, vision, pharmacy and dental plan year begins October 1, 2016, and goes through September 30, 2017.

The deductible and accruals toward maximum out-of-pocket reset October 1st. regardless of whether they were met the previous plan year.  However, if you did not meet your deductible during the previous plan year and had continuous coverage, any expenses applied to your deductible during the last 3 months of the previous plan year will be carried over and applied to the deductible for the new plan year.

The new plan year insurance rates that take effect October 1st will be reflected in the October 31st paychecks or for retirees from your bank account November 5th.  Please check your paycheck or bank statement to verify you have been charged the correct amount for your insurance elections.

Synergy Network Moda Medical Home (MMH) Election

Synergy Network/CCM (Coordinated Care Model) members enjoy team-based coordinated care and lower out-of-pocket costs and monthly premiums.  If you were enrolled in a Synergy Network plan for the 2015-16 plan year, your current MMH election should roll-over for the new plan year.

If you enrolled in a Synergy Network plan for the first time during open enrollment, you must select a Moda Medical Home (MMH) for each covered individual.  Each family member can choose a different MMH if they desire.

To choose your Moda Medical Home simply log into your MyModa account and choose the Medical Home tab or call Moda Customer Service at 1-866-923-0409l.

Note:  The 4J Wellness Clinic is a recognized Synergy provider and a Moda Medical Home.  If you would like to select the Wellness Clinic as your Moda Medical Home it is listed as:  Cascade Health Solutions at 200 N. Monroe Street, Eugene Oregon 97402 (the address is important to include)

Healthy Futures Program – Health Assessment due by October 15, 2016

If you agreed to participate in Healthy Futures during OEBB Open Enrollment, your reduced deductible takes effect the first day of the plan year, October 1, 2016.  At this point, if you have not yet completed your health assessment, you have 15 days to do so.  The Health Assessment is required each plan year in order to update your prior health answers.

To make sure you keep your incentive for the entire plan year, follow through on your promise and complete your Health Assessment (HA) between August 15 – October 15, 2016.

How to Complete a Health Assessment:  Completing the Health Assessment is easy and completely confidential — just follow the instructions below:

Moda Health members can go to www.modahealth.com/oebb and log in to your myModa account, or create a new account, if needed.

To create an account you will need your Subscriber ID that is listed on your Moda Health medical card. Once you’re logged in, click on the “myHealth” tab, then click the “Momentum, powered by Moda Health” link near the middle of the page, and then click the “Visit Momentum now” button at the bottom of the screen.

If you cover a spouse/partner on your medical plan, they need to complete their own health assessment as well and will need to do so through their own myModa account. Their Subscriber ID will be the same as yours, but they will need to create a unique user ID and password.

Note:  If you and/or your covered spouse/partner fail to complete the health assessment requirement within this year’s timeframe, you will lose your incentive retroactive to October 1, 2016. For assistance, call Moda Health at 866-923-0409.

Save the Date: November 15, 2016 – 403(b) / TSA Vendor Fair

The 403(b)/TSA Vendor Fair will be open to all 4J employee groups.   At the fair, employees will be able to visit individually with vendors and obtain information on retirement saving options.  November 15, 2016, 3:30 p.m. to 6:00 p.m.

Importance of District Email

Do you check your work email every day?   The District uses electronic messaging (email) to deliver essential communications and information portals to employees about district news, activities, training requirements, etc.

If you do not regularly read your work emails you may be missing out on news and updates relevant to your position and/or tasks that require action from you.

Flexible Spending Dates

The Flexible Spending Account (FSA) plan year begins October 1, 2016, and runs through September 30, 2017.  If you submitted an FSA enrollment for dependent care expenses and/or unreimbursed health expenses, you should see the first deduction come out of your end of October paycheck.

Moda Id Cards In The Mail?

Moda will mail new ID cards to members who either added or dropped dependents or changed Networks (Connexus/Synergy) during open enrollment.  They are not automatically sending out new ID cards to all members.  Old cards still will work as our member ID and group numbers have stayed the same.

If you would like a new medical or dental card, please log on to myModa and select “Request ID cards” from the member shortcuts panel. You also can contact Moda Health Customer Service at 1-866-923-0409
