4J Flu Shot Clinics Fall 2015
The U.S. Centers for Disease Control and Prevention (CDC) recommends that all people 6 months of age and older should get a flu vaccine, but there are different protocols for young children. Our particular vaccine is not approved for children under 10, so if you have children you would like to get vaccinated who are not eligible for the 4J flu shot clinics, you will need to contact your pediatrician.  The 2015-16 vaccine at the flu shot clinics will help protect against 4 different viruses: 2 A strains and 2 B strains – administered using prefilled syringes free of latex and thimerosol free. Three (3) flu shot clinics will be held in 4J for you to get this year’s vaccination.

Dress accordingly. The staff providing the vaccinations will need easy access to the upper arm.

4J flu shot clinic eligibility extends to:
* all active employees (even those who are not insurance eligible due to less than half-time status)
* all insurance eligible dependents of active employees, including those who have waived coverage
* all retirees and their dependents who are currently covered on the 4J insurance
* student teachers and substitutes (licensed and classified)

4J flu shot eligibility does not extend to:
*retirees who are no longer on the 4J insurance
*retirees’ dependents who are not on the 4J coverage (even when the retiree is still covered)
In this case, “dependents” refers to spouses and domestic partners, as well as insurance eligible children age 10 or over. All Children 10-16 years of age must have a parent or guardian with them to be vaccinated.

Other Flu Shot Options
If you can’t make the 4J flu shot clinics and have OEBB/Moda insurance, you can receive a flu shot at a network pharmacy – no copayment necessary!  Show your Moda Health ID card (or your ODS insurance card, which is still valid) to the pharmacist for billing before receiving a vaccine – otherwise it may not be covered.

The following participating chain pharmacies provide vaccines: Fred Meyer, Target, Bi-Mart, Haggen, Albertson/Savon, Rite Aid and Safeway. For a complete list of network pharmacies or if you have questions, call Moda Health Pharmacy Customer Service at 866-923-0411.

Cascade Health Solutions offers drop-in flu shots at their clinic at 2650 Suzanne Way, Suite 200, Eugene, OR from 7 a.m. to 6 p.m., Monday through Friday. No appointment necessary.

Rates and Deductibles 2015-16
The new medical, vision, pharmacy and dental plan year begins October 1, 2015, and goes through September 30, 2016.
The deductible and accruals toward maximum out-of-pocket reset October 1st. regardless of whether they were met the previous plan year. However, if you did not meet your deductible during the previous plan year and had continuous coverage, any expenses applied to your deductible during the last 3 months of the previous plan year will be carried over and applied to the deductible for the new plan year.

The new plan year insurance rates that take effect October 1st will be reflected in the October 31st paychecks or for retirees from your bank account November 5th. Please check your paycheck or bank statement to verify you have been charged the correct amount for your insurance elections.

2015-16 OEBB Healthy Futures
If you elected to participate in OEBB Healthy Futures, please remember to complete the online health assessment no later than October 15, 2015! To complete your health assessment, log into your myModa account, or create a new account, if needed.
Go to: www.modahealth.com/oebb. Once you’re logged in, click on the “Momentum, powered by Moda Health” link near the middle of the page.

   The District uses electronic messaging (email) to deliver essential communications and information portals to employees about district news, activities, training requirements, etc.  If you do not regularly read your work emails you may be missing out on news and updates relevant to your position and/or tasks that require action from you.

Remember the Tax Sheltered “Double
District-Match” 403(b) Retirement Savings?
I know – you’ve heard about it, but you’ve been too busy to look into it. Well, it’s still available for you!
Here’s the deal: The district will pay 1.25% of your monthly salary into a retirement savings account, if you commit to half that amount.  This “double-match” effectively triples your initial investment each month!
For every thousand dollars you earn,

You contribute:           $6.25
District contributes:    $12.50
Your investment:         $18.75

This is an example, using $1,000. The monthly contributions will always be exact percentages of whatever your monthly gross salary is.

To get started, contact one of the three (3) vendors eligible to receive classified contributions to open an account. Next, complete a District 4J Salary Reduction Agreement. Please make a copy for your records, and deliver the original to Financial Services at the Ed Center. Statewide Network or Synergy Network?
Moda Medical Home Election

If you enrolled in a Statewide Network plan, no action on your part is required.

If you enrolled in a Synergy Network plan for the first time during open enrollment, you must select a Moda Medical Home (MMH) for each covered individual. Each family member can choose a different MMH if they desire. Moda Health will be mailing information about medical home selection to new Synergy Network enrollees soon.

Contact Moda Health Customer Service at 866-923-0409 and confirm MMH selection with a Medical Customer Service Representative.

In order to change a medical home selection, members will need to select their new medical home and communicate the change to Moda Health using the option provided above. This change will be effective the first of the month following the notification of your new selection

If you were enrolled in a Synergy Network plan for the 2014-15 plan year, your current MMH election should roll-over for the new plan year.

New Moda Insurance ID Cards
Moda Health will be sending out new ID cards for all employees beginning October 1, 2015. If you need services prior to the arrival of your new cards, you can continue to use your existing cards.
If you are a new employee, an ID card will be sent to you upon enrollment in a Moda Health plan. If you need to access service prior to the arrival of your cards, please contact Diana McElhinney for assistance: mcelhinney_d@4j.lane.edu or 541-790-7679.

Flexible Spending Dates
The Flexible Spending Account (FSA) plan year begins October 1, 2015, and runs through September 30, 2016. If you submitted an FSA enrollment for dependent care expenses and/or unreimbursed health expenses, you should see the first deduction come out of your end of October paycheck.
