Flexible Spending Account Reimbursement Deadline
If you had a Flexible Spending Account (FSA) for unreimbursed health-related expenses and/or dependent care expenses during the plan year that ended September 30, 2014, please remember that all expenses must have been incurred between October 1, 2013 and September 30, 2014 to be eligible for reimbursement.
In order to claim any remaining unclaimed money you set aside for the previous plan year, you’ll need to either file your reimbursement claim electronically or submit a Reimbursement Request to PacificSource Administrators (PSA) along with the appropriate documentation. You can find information about how to file a request for reimbursement, as well as the necessary form(s) on the PSA website: http://psa.pacificsource.com/Flex/Request_Reimbursement/
The deadline for reimbursement requests for the previous plan year is December 31, 2014. If you have questions, please call PSA customer service at 541-485-7488 or 800-422-7038.
If you did not spend all of the funds you set aside in your FSA for unreimbursed health-related expenses during the 2013-14 plan year, remember that the US Treasury Department made a change in the FSA rules that allows you to roll over up to $500 of unspent funds.
Weight Watchers
Welcome to the holiday season! Remember that you and your dependents are eligible for Weight Watchers as part of our OEBB medical benefits. (Age restrictions may apply).

Wellness Clinic News
Winter Break:
The 4J Wellness Clinic will be closed December 24, 2014 through January 1, 2015. Regular clinic hours, which will resume January 2, 2015, are Monday through Friday, from 9:00 a.m. – 6:00 p.m. The clinic is closed for lunch from 1:00 – 2:00 p.m. If you will need a prescription refilled over winter break, please plan ahead. Remember; call your pharmacy for refills, not the clinic. The pharmacy will then contact your provider if necessary.
Eugene Urgent Care:
Eugene Urgent Care no longer offers a discount to 4J employees. OEBB coverage requires a $50 copayment per urgent care visit, deductible waived. Services that are referred out from Urgent Care may incur additional charges.

Moda & Quest Diagnostics
Choose the new Quest Diagnostics lab and pay $0 for lab services. At a Quest Diagnostics lab, you won’t have a co pay for covered services. For most members, the deductible is waived too. Here’s how to do it:
- Get tested at a Quest Diagnostic Patient Service Center*. Make an appointment at QuestDiagnostics.com/patient or by calling 888-277-8772.
- Ask your doctor to send your sample to Quest Diagnostics – or have Quest Diagnostics pick it up.

Severe Flu Season on the Way?
Early data suggests that the current 2014-15 flu season could be severe. The Centers for Disease Control and Prevention (CDC) urges immediate vaccination for anyone still unvaccinated this season and recommends prompt treatment with antiviral drugs for people at high risk of complications who develop the flu
So far this year, seasonal influenza A H3N2 viruses have been most common. There often are more severe flu illnesses, hospitalizations, and deaths during seasons when these viruses predominate.
If you have not yet gotten your flu vaccine, it’s not too late. If you have OEBB/Moda insurance, you can receive a flu shot at a network pharmacy – no copayment necessary.

There’s a lot you can do to prevent getting or spreading the flu. Here are a few ways to take action:
- Avoid touching your eyes, nose or mouth. Germs are often spread when a person touches something with germs, and then touches his or her eyes, nose or mouth.
- Cover your mouth and nose when coughing or sneezing. That way you won’t send germs into the air, where others can catch them.
- Wash your hands often. If you don’t have soap and water nearby, use an alcohol-based hand sanitizer.
- Get a flu shot. You’ll need a new one every year usually at the start of the flu season.
Should you find yourself with flu symptoms, please stay home to help prevent spreading the flu. Besides helping others stay healthy, you’ll also recover faster.
