2018-19 AHSAA Physical Form- Upload onto your DragonFly Account
All student-athletes must have a special AHSAA physical form completed and signed by a physician before the first practice and/or contest date. Each physical is good for one full year. Once completed upload to DragonFly account.
NFHS Sportsmanship Upload onto your DragonFly Account**Physical, Concussion, Waiver, Sportsmanship certificate and Birth Certificate all should be uploaded onto your DragonFly account to be eligible
