Student Records Available From Your School

To request your transcript or other student records:

• Grades K–8: For elementary school and middle school records, contact your school.

• Grades 9–12: Contact your high school registrar for records. If the records you are looking to obtain predate the records available at the school, you will be referred to the district office.

• Undocumented youth: If you are applying for consideration of deferred action for childhood arrivals, you need records dating back to June 15, 2007. There is a form to fill out and return to the district office.

Complete this form and return it to Paola Massingham, 4J Superintendent’s Office, 200 N. Monroe St., Eugene OR 97402. Fax: 541-790-7711. You will be contacted when your records are ready to pick up, or if more information is needed.
