Please register at the grade-level appropriate school or the one nearest to your home (applies to elementary students only), or the district office during the summer break. Records needed for registration include:

• Child’s birth certificate

• Child’s immunization forms*

• Emergency phone numbers (at least two)

• Name and phone number of your child’s doctor

• Parents’ cell and work numbers
