Resource CAFE (Connections and Access for Families through Education)
In February 2010, the Cabarrus Health Alliance received an Innovative Approaches grant from the North Carolina Division of Public Health, Children and Youth Branch, to work with community partners to improve the service system for children and youth with special health care needs and disabilities ages birth to 21. Cabarrus County was recently awarded funding for continuation of the initiative and expansion to Rowan and Union counties. The overall goal of Innovative Approaches is to foster improvement of community-wide systems of care for children and youth with special health care needs up to age 21 so that the following results occur:
- The service system effectively meets the needs of families with children and youth that have special health care needs.
- There is an increased satisfaction with services received.
- There are improved outcomes for children and youth with special health care needs.
THey are partnering with Family Support Network of Southern Piedmont to carry out this initiative in our community, in addition to community organizations providing services to these children, medical providers involved in their care, and most importantly parents of children and youth with special health care needs.
