The 12 th Milpitas High School Hall of Fame dinner will be held on Friday, March 10, 2017 at the Milpitas Community Center. Nominations are now open for the 2016 -2017 Hall of Fame inductees. Feel free to nominate a person who meets the following criteria. (Nominations will be open until Friday January 6, 2017.) Alumni (Include supporting evidence where applicable) ❑ Must have attended Milpitas High or Samuel Ayer for a minimum of two years. ❑ Graduated at least 5 years ago. ❑ Had a positive impact on students, faculty, and or community while attending Samuel Ayer or Milpitas High. ❑ Has been a respected contributor to school and/or community after graduation from Samuel Ayer or Milpitas High. Classified Employee/Certificated Employee/Administrator (Include supporting evidence where applicable) ❑ Has at least 10 years of service in the district. (Majority of service at Milpitas High or Samuel Ayer.) ❑ Is retired or no longer full-time employee in the district for at least 1 year. ❑ Was outstanding as a classroom teacher/classified employee/administrator. ❑ Always dedicated to the job, loyal to the school site and district (goes above and beyond). ❑ Acted as a leader and a mentor. ❑ Made significant contributions to school and/or community. ❑ Maintained high standards of personal integrity and ethical behavior. ❑ Has respect of community, colleagues, and students.
