(Student registration for students new for 2022-23 is ongoing for remainder of school year.) Please ENROLL YOUR CHILD ONLINE according to your resident school, or Randall Elementary, if you are interested in the Dual Immersion program.

NEW STUDENTS ONLINE ENROLLMENT
Students currently enrolled in MUSD do not need to enroll again.

No Mortgage: Submit ONE of the following:
- Yearly Tax Assessment Post Card
- Bi-annual Property Tax Bill
C. ONE of the following ORIGINAL Utility Bills:
- Current PG&E bill OR
- Current City of Milpitas garbage/water bill
*TWO of the following if parent/legal guardian is not property owner or primary renter:
- Original paycheck or paystub issued by employer
- Correspondence from government agency (WIC, Medi-Cal, Social Security, Unemployment)
- Valid vehicle registration (not pink slip)
- Current monthly bills (bank loan, credit card, physician/dental statement, bank statement)
- Or to be determined by District Office Representative. District Office has the right to accept alternative forms of proofs of residence on a case by case basis.

3. PROOF OF TB RISK ASSESSMENT DUE AT REGISTRATION
TB Assessment is required for TK or K, and new students regardless whether they have traveled or not. For returning students if traveled outside of Santa Clara County for 12 months or more, Santa Clara County TB Risk Assessment form must be filled out and signed by medical provider in USA within 12 months of school entry.

- IGRA Blood test results: negative or positive (repeat if test indeterminate)
- TTD/PPD skin test, which must indicate: 1) Date given, 2) date read, 3) results in mm, 4) positive or negative, 5) Medical Provider’s signature or stamp.

4. PROOF OF CURRENT IMMUNIZATIONS REQUIREMENT FOR REGISTRATION: ALL GRADES*
- Polio: Age 4-6: 4 doses, 3 ok if one given after age 4: age 7 and older: 4 doses, 3 ok if one given after age 2
- DTAP/DTP/DT: AGE 4-6: 5 doses, 4 ok if one given after age 4. Age 7 and older: 4 doses, 3 ok if one given after age 2
- TDAP: 7th grade: 1 dose, must be given after age 7
- MMR: age 4-6: 2 doses, must be given after age 1
- Hepatitis B: 3 doses
- Varicella: 2 doses
Medical Exemption:
As of Jan 1, 2021, a law enacted by Senate Bills 276 and 714 requires new medical exemptions to be issued using a State database and a standardized medical exemption form. Immunization exemption letters are no longer acceptable.
Medical exemptions issued using the California Immunization Registry-Medical Exemption website (CAIR-ME) are the only medical exemptions schools and pre-kindergarten facilities can accept from students entering a new grade span in 2021 and beyond. These medical exemptions are subject to CDPH review and may be revoked. Schools, child care facilities and parents will be notified if an exemption is revoked.

5. TK or kinder dental oral health exam
TK-Kinder requirement and 1st Graders with no previous school experience: The Oral Health Exam can be performed by your child’s dentist up to 12 mos. prior to school entry and the completed Oral Health Assessment Form is due to your child’s school no later than May 31 of your child's first school year.

6. first grade requirement Physical Exam (CHDP)
1st Graders must have a completed Physical Exam (CHDP) 18 months before or up to 90 days after enrolling in first grade. No insurance? Call 1-800-689-6669 for free CHDP info. Full enrollment will be PENDING until the completion of the physical exam.

7. SPECIAL EDUCATION INFORMATION (IF APPLICABLE)

8. additional Information
- Enrollment is completed at your resident/home school as determined by your address.
- PO Boxes and business addresses may not be used as your student’s residential address.
- Additional proofs of residency or forms may be required based on specific living conditions and all documents submitted must be current and valid.
- Any required Affidavits must be completed at your home school before enrollment is complete. Your student will be enrolled after completing and submitting online registration and/or after all of the above requirements have been verified at your home school office.
- All new incoming transitional kindergarten through 4th grade students for the 2022-2023 school year choosing to opt out of Randall's Spanish Dual Immersion program will automatically be assigned to Rose Elementary, but will still need to complete enrollment at Randall school office. Student files will be forwarded to the new resident school, Rose Elementary.
- First day of school is Wednesday , August 10, 2022. NOTE: (For elementary students grades TK-6) Due to highly impacted enrollment in Milpitas Unified, students who do not attend class the first day of school will lose placement in the class/school and may be assigned to another school within the District. NO EXCEPTIONS.

You must register your child at your resident school based on your Milpitas resident address. Kindergarten Enrollment 2023-2024
Kindergarten and Transitional Kindergarten registration begins on January 24, 2023. Please begin the register process ONLINE. Current TK students will automatically be enrolled in kindergarten at their resident school.
Mattos Enrollment Limitations
March 1st marks the first day of enrollment for all Milpitas Unified School District families. The limited capacity of the current facilities at Mattos Elementary and the constraints of not being able to have K-1st grade students on the second floor presents a challenge in our ability to accommodate all new families wishing to enroll in Mattos.
We will continue to adhere to the interim boundaries until Mattos’ facilities reach full completion, at which time we will be able enroll students at Mattos per the board established boundary lines. During this interim period, the 2019-2020 SchoolFinder will assign all incoming NEW students with addresses 600-800 S. Abel Street and 1001 S. Main Street to Zanker in all elementary grades for 2018-19 and 2019-20. All Mattos students currently enrolled in grades K-2 at Mattos will remain at Mattos.
In order to accommodate the enrollment process in an equitable manner, we will hold a lottery for the limited amount of seats. The lottery process will take place March 8, 2019 and parents will be notified of final results by March 13, 2019. Families living within these boundaries can submit their registration paperwork from March 1-March 8. Anyone who submits their completed registration paperwork by 3:00 on March 8 will be entered into the lottery system. A letter was mailed February 1, 2019 to all Mattos’ boundary residents informing them of the process. In addition, Mattos and District staff will hold a Community Informational Forum on Feb 11, 2019 at 5 p.m. at Mattos Elementary School.

HOW DO I ENROLL MY CHILD IN KINDERGARTEN OR TK?

WHAT IS THE CURRICULUM?
The TK and kindergarten curriculum are both developmentally appropriate and focus on literacy and mathematics along with a strong emphasis on exploration, self regulation, social, and emotional engagement. The California Common Core & State Standards and the Next Generation Science Standards guide our kindergarten program. The Milpitas Unified TK Content Standards provide a bridge between California Preschool and Kindergarten standards to support the needs of the TK student.
WHICH SCHOOLS ARE OFFERING TK?
For the 2023-24 school year, TK sites will be announced in January 2023. TK is currently being offered at the following sites: Burnett, Mattos, Pomeroy, Randall, Rose, Spangler, and Zanker.

WHAT IS A DEVELOPMENTAL KINDERGARTEN PROGRAM?
It is a two year kindergarten program designed to build a solid foundation for success in school. TK provides a bridge for our young five year olds between the flexible, exploration based environment of preschool and the more structured academic setting of kindergarten. It promotes each child’s progress in all areas of development. Much care is given to providing a rich, stimulating environment of active learning where each child develops and practices the physical, social, and emotional skills that naturally accompany a child’s intellectual growth.

Age Enrollment & Grade Placement (TK - 12)
Note: All new incoming transitional kindergarten, kindergarten and 1st grade students choosing to opt out of Randall's Spanish Dual Immersion program will automatically be assigned to Rose Elementary, but will still need to complete enrollment through Randall school. It is recognized that students do their best work in school when they are placed in a grade with other students of the same chronological age. Initial grade placement of enrolling students shall be based on chronological age, unless the student is transferring from another public school districts in U.S., case initial placement will be at the grade level the student would have been in at his/her prior school.
If the grade listed in the below chart is different from that prescribed by your student’s current or past school grade, parents must bring evidence to the registration of successful grade completion, or existing grade enrollment, from previous schools (such as a report card, or an existing enrollment report, or transcripts, etc.).
If no evidence is provided, the student will be placed in the grade listed in the below chart at the time of registration. Any subsequent grade change must be discussed with the MUSD school in which your child is enrolled after registration.
Other Countries/ USA Private Schools
Students who come from other countries, who come from private schools within the United States, or who have not been in school for two or more years, are initially placed according to their age. If a student has completed the grade corresponding to chronological age at a private school, MUSD school team will request supporting evidence that student has successfully completed the grade and will follow the district acceleration process.
Grades 9-12
In addition to the criteria above, MUSD will also consider credits earned toward graduation, transferability of graduation credits from the student’s prior school, and observation and evaluation by classroom teachers and guidance personnel.
Grade Placement will be determined based on the best interest of the student and will consider academic and social-emotional development.
Any subsequent grade change must be directly discussed with the MUSD school administration.

PUPILS OF MILITARY FAMILIES: Assembly Bill 2949 - Pupil Residency
Existing law provides an exception to the residency requirements for a student whose parent is transferred to or is pending transfer to a military installation within the state while on active military duty pursuant to an official military order. AB 2949 adds section 48204.6 to the Education Code to require a local educational agency to permit a student who meets certain criteria to continue attending the student's "school of origin" during that school year, regardless of any change of residence of the military family. If the student's status changes due to end of military service, then the student is allowed to stay in the school of origin for the remainder of the academic year if he or she is between grades 1-8, or through graduation if the student is enrolled in high school. The new law effectively mirrors the laws related to protecting foster youth and homeless children, who are also susceptible to frequent school changes.
