Hello there! Welcome to Northwest Middle! I'm Nurse Turpin.If your child has medication that they will be taking at school ( ADHD, allergy, Epi Pens, inhalers, etc) please make sure that you have the completed forms. The forms can be found under the forms sections. These forms REQUIRE a MD signature as well as a parent signuate. 7th graders are REQUIRED by the state  to have DTAP & Meningo vaccines.
