Referral

Each school site utilizes a Student Success Team to help assist any student who is having academic, behavioral, or other difficulties at school. Within this process, a history of the student is obtained and interventions to assist the student are implemented and documented over a period of time. If these interventions fail to remediate the difficulties, a referral may be made for a special education assessment.

Identification

If a parent or Student Success Team makes a direct referral to the school site or district office, the referral will be sent to the Special Education Department. Within 15 days of receipt of the request for assessment, the parent will receive either an assessment plan that outlines the proposed assessment or a Prior Written Notice outlining the reasons for denying the request for assessment. Once the assessment plan is signed, the district has 60 days to complete the assessment and hold an Individual Education Program meeting to review the results. This meeting includes the parents who are essential members of the team. Should the student meet one of the fourteen eligibility categories, an Individual Education Program describing supports and services will be written and implemented by the team. Once the team agrees to special education, the services cannot be changed unless the team reconvenes and develops a new plan based on the current disability and present levels of performance.
