Attention: All students entering the 7th grade must have two additional shots. For you to comply with the State of Tennessee, we need proof of these shots from the health department or your local health care provider before the start of the 2024-25 school year.  All immunizations must be on the TN Department of Health form - any other version will not be accepted per school board policy. Your 7th grader will not be allowed to attend school until this form has been submitted.
