Registration Forms

Please use the downloadable attachment at the bottom of the page to register for any enrichment course.  If you are registering for more than three courses, please use another form to complete the registration process.  Make sure to complete all sections and include your check or money order for the total amount of classes purchased.

There are now four ways you can register: 

1) Register securely by credit card or check any time using our new online catalog.

2) Mail your registration to:

3) Fax your registration to Karyn Pheiffer at 203.468.4086

4) Drop off your registration to or office between 8:00 AM - 1:00 PM, Monday - Friday

CDP, ESL, NEDP, ABE and Citizenship Registration Forms

Below are the registration forms needed to be completed to join one of the mandated classes provided by East Haven Adult Education.  They include the Credit Diploma, GED,  National External Diploma, Adult Basic Education and Citizenship programs.  Please download them, fill them out completely (or as best you can) and bring them in when you come.

A continuación se encuentran los formularios de registro que se deben completar para unirse a una de las clases obligatorias proporcionadas por East Haven Adult Education. Incluyen el Diploma de Crédito, GED, Diploma Externo Nacional, Educación Básica para Adultos y programas de Ciudadanía. Descárguelos, llénelos completamente (o lo mejor que pueda) y tráigalos cuando venga.
