Most physicians have a copy of the TSSAA physical form.  However, one is included on this page for your convenience.

Free or reduced fee athletic physicals may be available from local practitioners and clinics for students wishing to participate in athletics for the upcoming school year.  Availability of such is dependent on local practitioners and clinics' ability to provide these services.  Sports physicals are not offered at any individual school.  

The sports physicals received on or after April 15th are good for the entire next school year.  TSSAA requires athletes to receive a sports physical on April 15th or after, even to try out for any school sport.
