2022-23 School Year Information

All students should have a completed 2022-2023 Student Health Information form.

Medication forms and/or Health plans should be completed as soon as possible so they will be on file with me the first day we return to face to face learning. Medication Authorization forms have to be renewed each school year. There is a link on my page for documents or you can access via Cabarrus County Schools.

Immunization Records for all students should be up to date and on file at school in order to attend school. All seniors are required to have a booster of Meningo vaccine within the first 30 days of their senior year.

NC Health Assessments are required to be on file for any student new to NC Public Schools.

***** Seniors: As of August 2020, NC law requires all 12th graders to have a meningitis vaccine booster dose in order to attend school. If you were born on or after 1/1/2003, this applies to you. Parents, please provide the school with a copy of your updated record.
