Student Registration Documents 

• Student Registration Packet (Please provide phone/fax number of former school)
• Change of Address Form (Only applies if you have moved within the South Colonie Central School District)
• Affidavit (Only applies if parent/guardian lives in dwelling that they do not lease or own in their name)
• Migrant Education Outreach Program Registration (Only applies if you have moved within the last three years to obtain employment in agricultural, food processing, or farm work)
