School Health Examinations
Mandated School Health Examinations
New York State Education Law requires that all new students and all students in grades Pre-K, Kindergarten, Grade 1, Grade 3, Grade 5, Grade 7, Grade 9 and Grade 11 complete a school health examination for the school year. The Chief Medical Officer will provide these examinations unless a private examination is completed and submitted to the child's Health Office for approval.
Mandated Interscholastic Sports School Health Examinations
New York Sate Education Law requires that all students participating in interscholastic sports in grades 7 through 12 complete an annual school health examination and an updated medical history for each sports season. The Chief Medical Officer will provided these examinations unless a private examination is completed and submitted to the child's Health office for approval.
*Please note that all interscholastic health examination completed forms must be submitted to a child's Health Office at least two weeks before the beginning of the season for approval by the Chief Medical Officer.*
