Pre-Kindergarten and Kindergarten
STATE OF TENNESSEE SCHOOL ENTRY REQUIREMENTS
* To enroll in Kindergarten, your child(ren) must be five years of age on or by August 15.Upon school entry, the following records are required:- A record of birth
- A recent physical (completed in current year)
- Maury County Public Schools also requires three (3) proofs of residence. (Examples of proof: sales contract, rental agreement, power or gas bill, cable bill, driver’s license with correct address, voter’s registration, change of address from post office, payroll stub, etc.)
- *Documentation of Immunization for:
TN Immunization Requirements for Children Enrolling in Pre-Kindergarten:• Diphtheria-Tetanus-Pertussis (DTaP, or DT if appropriate)
• Poliomyelitis (IPV or OPV)
• Measles, Mumps, Rubella (1 dose of each, usually given together as MMR)
• Varicella (1 dose or history of disease)
• Haemophilus influenzae type B (Hib): age younger than 5 years only
• Hepatitis B (HBV) (July 1, 2010)
• Pneumococcal conjugate vaccine (PCV): age younger than 5 years only (July 1, 2010)
• Hepatitis A: 1 dose, required by 18 months of age or older (July 1, 2010)
