Registration Information

  • To register your student at Battle Creek Elementary, please stop by the office with the following information.  
    Please bring with you the following items:

    • Record of birth
    • Record of current Medical Examination (completed in the current year)
    • Tennessee Certificate of Immunizations
    • Social Security Card
    • 3 Proofs of address (i.e. driver's license, utility, lease, voter registration card, etc.)

    TN Immunization Requirements:

    • Diphtheria-Tetanus-Pertussis (DTap, or DT if appropriate)
    • Hepatitis B (HBV)
    • Measles, Mumps, Rubella (2 doses each, usually gives together as MMR)
    • Poliomyelitis (IPV or OPV): final dose on or after the 4th birthday now required
    • Varicella (2 doses or history of disease)
    • Hepatitis A: total of 2 doses, spaced at least 6 months apart

    Items Accepted for Proof of Residence

    Please provide three (3) of the following 17 items, showing the parent/guardian’s name and address, to prove residence. 

    1. Sales Contract
    2. Real Estate Tax receipt
    3. Mortgage information (contingency agreements not accepted)
    4. A utility bill or turn-on receipt
    5. A rental contract
    6. Voter’s registration card (issued at least 30 days prior)
    7. H.S. Family Assistance Application or Recertification Interview Card
    8. Health Card/Medical Card
    9. A credit card or credit card company statement (dated prior to registration)
    10. Recent Income Tax records – State or Federal
    11. Recent bank statement – checking or savings (dated prior to registration)
    12. A letter from an employer on the employer’s stationery
    13. A recent employment ID card or union card bearing current address
    14. Photo copy of change of address Postal Form PS 35-75 and 3 pieces of processed mail with yellow change of address strip to new address
    15. A recent payroll check
    16. Driver’s license (Issued at least 30 days prior)
    17. Car tag registration

    If the above documentation is impossible to provide in a timely manner, enrollment may be approved by the Director of Pupil Services or the Superintendent of Schools.

    Exception:  Students In Transition (Homeless Students) - contact Pupil Services
