Reporting Absences
Attendance is very important to a student’s overall success at Clark Magnet. We have an automated dialer system calling home in the late afternoon and early evening to inform parents of student absences. When a student is absent, the parent/guardian must call (818) 248-8324 as early in the day as possible on the day of the absence. Only a parent or legal court-appointed guardian can report an absence. Absences must be cleared within three school days of the first day of absence. If you call before 7:45 am or after 4:15 pm or on a weekend or holiday, leave a message on extension 1220. Be sure to speak slowly and give your name, the student’s full name and I.D. number, the date of the absence and the reason for the absence. Notes for absences or early dismissal from school are not accepted. We must speak with a parent or guardian.
Please schedule appointments after school hours. If a student must leave school during the day, the parent/guardian must call (818) 248-8324 before the student is to be dismissed. The student must then pick up a “Permit to Leave School During School Hours” form from the main office. Please call as early in the day as possible to allow enough time for the student to pick up his/her “Permit to Leave School During School Hours” form. Students who leave school without checking out through the office are considered truant for the time they miss class. The absence cannot be cleared after the student leaves the grounds.
Doctor Notes
If your doctor has ordered your student to be on crutches, wearing a cast of any kind, or a special supportive leg/knee brace, or boot a doctor's note must be submitted to the LVN Ms. Antig, in the Health Office. It must indicate for how long the device is to be worn or used and any specific limitations. Without the doctor’s note, you will be called to pick your son/daughter up from school, no exceptions. A doctor’s note is required for your student to return to school if he/she has been absent for five days or more.
Medications
If your student needs to take any medication during school hours, a Request for Medication to be Taken During School Hours and/or the Self-Administration of Prescribed Medication (Inhaled) form(s) must be completed by your doctor and submitted to the LVN, Ms. Antig, in the health office. This includes all over-the-counter medications as well as those prescribed by your doctor. All medications will be kept locked in the health office.SL 12/21/21
