In order to withdraw a student from the Collinsville School District, a parent/guardian should follow the steps outlined below:
Elementary Schools and Dorris Intermediate School
1. The parent or legal guardian of the student should visit the main office of the current school to inform the office staff of the intended withdrawal or transfer. The following information should be provided in writing:
- Student’s full name
- Reason for withdrawal (transfer, move out of state, etc.)
- Forwarding address (if known)
- Name, address and telephone number of the transfer school that the student will be attending (if known)
2. Textbooks, library books, athletic uniforms and any other school-owned property that has been assigned to the student must be returned to the school at the time of withdrawal or transfer notification.
3. Financial obligations to the district (including, but not limited to, unpaid fees, overdue library books and/or lost textbooks) must be settled.
Note: During the enrollment process at the new school, be sure to complete the necessary forms to request that your child’s Collinsville School District records be transferred to the new district. If transferring to an Illinois public school, have your child’s current Collinsville school complete the Student Transfer Form (available in the Media Library above) and fax it to the school your child will be transferring to.
Collinsville Middle School
1. The parent or legal guardian of the student should visit the main office of the current school to inform the office staff of the intended withdrawal or transfer. The following information should be provided in writing:
- Student’s full name
- Reason for withdrawal (transfer, move out of state, etc.)
- Forwarding address (if known)
- Name, address and telephone number of the transfer school that the student will be attending (if known)
2. A withdrawal note will be provided. The note should be presented to each of the following staff members and then returned to the office:
- Teachers (each teacher will provide a signature on the form to verify receipt of any school textbooks or other materials loaned to the student).
- Librarian/Media Specialist (the librarian/media specialist will provide a signature to indicate library books have been returned and/or overdue fines have been paid).
3. Financial obligations to the district (including, but not limited to, unpaid fees, overdue library books and/or lost textbooks) must be settled. Athletic uniforms and any other school-owned property that has been assigned to the student must be returned to the school at the time of withdrawal or transfer notification.

- Name, address and phone number of the transfer school that the student will be attending (if known)
2. A withdrawal note will be provided. The note should be presented to each of the following staff members and then returned to the Guidance Department Office:
- Teachers (each teacher will provide a signature on the form to verify receipt of any school textbooks or other materials loaned to the student).
- Librarian/Media Specialist (A signature will be provided to indicate library books have been returned and/or overdue fines have been paid).
- Registrar (a signature will be supplied after financial obligations such as unpaid fees, lost textbooks, etc. are settled). Athletic uniforms and any other school-owned property that has been assigned to the student must be returned to the school at the time of withdrawal or transfer notification. A parent/guardian may request a partial refund of fees using the Student Instructional Material Fee Refund Form (located in the Media Library above) if the account is in good standing at the time of the withdrawal/transfer.
- Attendance Secretary (a signature will be provided, along with information about the student’s attendance up to the point of withdrawal/transfer).
Note: During the enrollment process at the new school, be sure to complete the necessary forms to request that your child’s Collinsville School District records be transferred to the new district.
