April 14, 2021 – The following announcement from the Madison County Health Department has important information for students 16+ regarding the COVID-19 vaccine:
Madison County eligibility to receive the COVID-19 vaccine is open to anyone 16 years of age or older who lives or works in Illinois. We encourage everyone who is eligible to get the COVID-19 vaccine as soon as possible to help slow the spread and increase protection against this new virus.
Appointment dates and times are available at various locations in Madison County. To schedule your vaccination appointment, visit www.madisonchd.org and click on the green Appointments are Open link at the top of the page. Follow the prompts and select the date, time, and location that you want and then schedule your appointment. In the end, you will receive a confirmation and a QR code. Print the QR code and bring it with you to your appointment. If you need assistance scheduling your appointment or do not have access to a computer or the internet, call (618) 650-8445 Monday-Friday 8:30am-4:30pm and someone will schedule your appointment over the phone.
What to bring to your appointment:
- Photo ID with birth date for all ages.
- If using a school photo ID, bring birth certificate.
- Parent or guardian (with proof of guardianship) if 16 or 17 years old.
- Proof of employment or residency in Illinois.
- QR code from scheduling your appointment.
- Vaccination card for 2nd dose appointment.
