2015 2016 Student Registration
Kindergarten – 12th Grade
Student Registration is now more convenient than ever before. All families will register their children on-line using their own personal computer or device. Families that are new to the Collinsville Unit #10 School District, have changed their address or require an affidavit or homeless application are required to complete the enrollment process in person at the Enrollment office located at 123 W. Clay, Collinsville, IL.
New this year, if your child is currently enrolled in the Collinsville School District, resides at the same location from which he/she was enrolled last year and will be in grades Kindergarten – 12th next year, they will be able to take advantage of the on-line registration process. If discrepancies in enrollment information are found, students will be blocked from on-line registration until updated residency information has been provided at the Student Services office at 123 W. Clay. Additionally, you must have a copy of the student’s birth certificate on file to complete the on-line registration process.
Online Registration Procedure
Go to www.kahoks.org , click on on-line student registration. You will need an e-mail address and your students ID # in order to proceed with the on-line process. The student ID # is the number that your student uses to purchase their lunches and can be found on the address page of this mailing. On-line registration will open Monday, July 13, 2015 and close on Wednesday, August 12, 2015 at 7:00 p.m., so please take advantage of this process and avoid the registration lines.

Collinsville High School (Grades 9-12) students will need to pick up their schedules and books during Centralized Registration (see attached schedule). During this time students will also have their picture taken, pick up P.E. uniforms and request parking permits. Please print the final page of your on-line registration as Proof of Registration. When you present the Proof of Registration you will be directed to the CHS area to complete the registration process.
Collinsville Middle School Students (Grades 7 & 8) will need to pick up their schedules at CMS on August 5 (8am-12pm and 1pm-3pm) or August 6 (1pm-7pm).
Dorris Intermediate School (Grades 5 & 6) and all Elementary Schools will post class lists on the front entrance to each building on August 5 after 3:30pm.
To complete enrollment come to the Enrollment Office at 123 West Clay St., Collinsville, IL, phone 618-346-6350 ext. 4237 or ext. 4228 or attend the Unit #10 Centralized Registration (see information below). The following families will not be able to complete on-line registration until enrollment information is complete
Families who are new to the District
Families with a new address, different than the address submitted during enrollment.
Kindergarten students who have not completed the enrollment process
Early Childhood Students
Pre-Kindergarten students
Families who have a Homeless or Affidavit application
Families that have not provided a copy of the studentåÕs birth certificate
All new students to the Collinsville CUSD #10 School District and those with address changes must prove residency to enroll. (The district reserves the right to verify all residency information.)

Families that have children with different last names can pick one day to bring all the children at the same time
For new students to the Collinsville School District or students that have an address changed, a custodial parent or legal guardian must bring in the following to complete the registration:
1. Proof of residency (Guidelines to establish student residency can be found on our District webpage at www.kahoks.org ).
2. Certified Birth Certificate and Social Security number.
3. Completed free and reduced lunch application.
4. Notarized Affidavit-(if applicable).
5. Updated Student Enrollment Verification, Transportation, and Handbook Acknowledgment Form provided by district).
6. Updated Student Health Information Form.
7. Any court documents pertaining to custody of the child (if applicable).
School Starts August 11, 2015.

7:55 a.m. – 2:55 p.m.
*Please note: students will be dismissed 30 minutes earlier every Wednesday during the school year.
