In compliance with North Carolina law (General Statute 130A-152-157), parents/guardian shall ensure a child has received the required immunizations at the age required by law on or before the first day a student enters school. It is the responsibility of the parent/guardian to provide the immunization record no later that 30 days after the child enters school. NC Immunization requirements for School Entry
All students entering school need the following vaccines:
Immunization
Dosage
DTaP (diphtheria, tetanus, and acellular pertussis)
5 doses( last dose must be on or after age 4)
Polio
4 doses( last dose must be on or after age 4)
Hib (haemophilus influenza type B)
3-4 doses
MMR (measles , mumps and rubella
2 doses ( first dose must be on or after age 1)
Hepatitis B
3 doses ( last does must be after 24 weeks of age)
Varicella (chicken pox)
2 doses if entering kindergarten on or after 07/01/15. 1 dose if entered school before 07/01/15. (1st dose must be on or after age 1)
Students entering 7th grade on or after 07/01/15 must have the following vaccines
Tdap (tetanus, diphtheria, acellular pertussis)
1 booster dose
Meningitis( meningococcal)
1 dose
