    IMPORTANT HEALTH INFORMATION 

     
    Immunizations/Vaccines:

    In compliance with North Carolina law (General Statute 130A-152-157), parents/guardian shall ensure a child has received the required immunizations at the age required by law on or before the first day a student enters school. It is the responsibility of the parent/guardian to provide the immunization record no later that 30 days after the child enters school.

     

    ALL students are required to have:
    Dtaps - 4 or 5 but last one must be on or after 4th birthday
    MMR - 2 and 1st one must be on or after 1st birthday
    HepB - 3 and last one must be on or after 24 weeks of age
    Varicella - 1 on or after 1st birthday if born after 04/01/01
    HIB - not required after age 5
     
    Kindergarteners-1st Grade (students who began school on or after 07/01/15):
    Polios - 3 (if at least 6 months between 2nd and 3rd dose) or 4 but last one must be on or after 4th birthday
    Varicella - must have a 2nd varicella at least 28 days after the 1st one
     
    2nd-12th Grades:
    Polios - 3 if last one is on or after 4th birthday or 4 total
     
    7th-12th Grades:
    Tdap - 1 booster or at age 12 whichever comes first
    Meningitis - one dose is required for individuals entering the 7th grade or by 12 years of age, whichever comes first, on or after July 1, 2015.
    *An individual born before January 1, 2003 shall not be required to receive a meningococcal conjugate vaccine.  You do not need to worry about 2nd booster until 2020.
     

     

    Physical Exam/Health Assessments:

     

    Effective July 1, 2016 General Statute 130A-440; 10A NCAC09.3005 states that every child entering in NC public schools shall receive a health assessment. The health assessment shall be made no more than 12 months prior to the date of school entry. The parent, guardian, or responsible person shall have 30 calendar days from the first day of attendance to present the required health assessment transmittal form for the child. The state health assessment must be on the current NC Kindergarten Health assessment form.
     
    Garrett's Law:
    Senate Bill 260, previously referred to as "Garrett's Law", requires each school system to provide parents and guardians with information about meningococcal meningitis, influenza diseases, HPV and their vaccines.  See the information below:
