Transition planning is a process that is designed to provide students with
disabilities the necessary skills and services to smoothly transition from
school into adult life.
Using this Page
Transitioning into the adult world can be a challenge for all young people, even more so for students with disabilities. This website is designed to provide resources and support to help students and family members navigate this transition planning process.
The transition planning process should be initiated at an early age. In Massachusetts Public Schools, the transition planning process begins when a student turns 14. At this point, a transition plan is created and remains a working document that is built into the student’s Individualized Education Plan (IEP). As the student gets closer to graduation, the student’s team will collaborate on how best to prepare for adult life.
