(Preschool thru Grade 12 // Excel Tecc & Cevec Programs)

2025-2026 School Year Registration is now open for all grade levels including Accepted Non-Mayfield Excel TECC and CEVEC students.

Identify your current family status and select the

appropriate box below to begin registration.

The ANNUAL UPDATE is mandatory for all EXISTING students (and those enrolled before 7/1/25) to be completed within your parent portal before the first day of school.

If you need to enroll a new student to your current family, you may do so within the Annual Update in the Student tab.

Select Family Type to begin registration

EXISTING MAYFIELD FAMILIES - Families with existing student(s) currently enrolled in Mayfield, and adding a new student

NEW MAYFIELD FAMILIES - New Mayfield residential parent/guardian registering student(s) for the first time (no students currently in the district)

NEW NON-MAYFIELD FAMILIES - Non-Mayfield students that have been accepted into an Excel TECC or CEVEC program

Registration Forms

Required Documents Needed

A list of documents that are needed to be scanned and saved to your computer in order to complete the online registration.

Residency Affidavit

Complete this form if living in home without a lease. Please return notarized form to the Board of Education.

Authorization for Release of School Records

Please fill out this form if you are transferring from another district.

Intra-District Open Enrollment/Transfer Form

Complete this form if you are a resident of the district and would like to request that your student(s) attend a different elementary school than the one to which your address is assigned. Deadline for consideration is Aug 1 at 3:00 PM.

Language Usage Survey

If any language other than English is spoken in the home and/or by the child, the first page of this form must be completed by the parent/guardian.

This form must be filled out and saved in order to upload completed form into the Online Registration.

Preschool Physical Form

An updated physical exam is required to be on file every 13 months for children to attend preschool.
