HEALTH SCREENINGS:
State mandated health appraisals (physicals) must be completed after June 1st of the next school year for New Registered Students, Kindergarten, Grades 1, 3, 5, 7, 9, and 11.
All physicals and sports physicals must be completed by a NYS licensed physician, nurse practitioner or physician assistant.
The school physician will give these exams if the health office does not receive the completed form.
SPORTS PHYSICALS:
Sports physicals completed by your health care provider must be completed after June 1st of the next school year. Forms that do not have blood pressure will not be accepted.
The school doctor will be at the school to perform sports physicals on the following dates:

Thursday, June 22 Girls 3:30 p.m. / Boys 4:15 p.m.

Thursday, July 13 Girls 9:00 a.m. / Boys 9:45 a.m.

Thursday, August 17 Girls 9:00 a.m. / Boys 9:45 a.m.
NOTE: Athletes should be prepared to stay for the entire session.
A Summer Physical Examination Letter will be mailed in June to students entering grades 7-12 for the upcoming school year.
