Work Permit Information

MAYFIELD CITY SCHOOLS
STUDENT INSTRUCTIONS FOR OBTAINING WORK PERMIT AGES 14-17

1- Fill out APPLICATION FOR EMPLOYMENT CERTIFICATE.
Have your parent/guardian sign on the signature line.

2- Have your employer complete PLEDGE OF EMPLOYER FORM.
Employer signature, address, date, Employers Tax Payer ID Number, phone number, days worked, hours worked: all fields are required. (This form is mandatory to get a work permit)

3-Have your physician complete PHYSICIAN’S CERTIFICATE.
Athletic physical on file with the Athletic Department will be acceptable.

4-Email or bring in all three forms to Laura Kraft (MHS Guidance Office)
 6116 Wilson Mills Rd 
*Please note during the summer these will be processed once a week. You must email the information as the office will be closed. 

The forms can be found on www.mayfieldschools.org under the high school menu. Each change of employment requires a new work permit
