Enrollment Forms

  • Application for Admission to CCC&TI for General Population
    This will open the general application to enroll at CCC & TI. This application should be completed if you are a high school senior who has never taken any Huskins or Dual Enrollment classes with CCC & TI. Be sure to print out the signature page after completing the application. The signature page should be signed and turned in to your school's Transition Advisor.
    Application for Admission to CCC&TI for High School Programs
    All high school students wishing to participate in any Cooperative Program with CCC & TI must complete this application and submit it to your school's Transition Advisor.
    Continuing Education Student Application Registration Form - Page 1 of 2
    Students wishing to enroll in a Continuing Education class must complete this application, along with the Petition for the Admission of a Minor Applicant to Continuing Education form. As with all other CCC & TI college forms, please submit this form to your high school's Transition Advisor.

    Dual Enrollment Permission Form
    Each high school student (16 years old or older), wishing to take a Dual Enrollment class must complete this form and turn it in to your school's Transition Advisor. You must also complete the Application for Admission, High School Programs version and turn it in to your Transition Advisor along with the Huskins form.
    Permission for the Admission of a Minor Applicant to Continuing Education
    Students who are 16 or 17 years old, wishing to take courses at CCC & TI through the Continuing Education division, must complete this minor permission form, along with the Continuing Education Student Application Form, and submit it to your school's Transition Advisor.
    Request for High School or College Transcript
    High school graduates wishing to attend CCC & TI must submit official transcripts to the college. Submit this form to your high school.
