Return to Activity

  • We also wanted to remind you of our return to activity protocol for students who test positive for COVID. When a student is released from isolation, they will be able to resume normal daily school activities. Before they can resume physical activities:

    • If they’re participating in a school sport: They will need to follow up with their healthcare provider following this COVID infection. Before they can return to physical activity/sports, a post-COVID cardiac assessment by their primary care provider is required and there may be a gradual return to play before returning to full activity. Let your school nurse know if you need more information on this or if the PCP needs a blank post-COVID cardiac assessment form.
    • If they’re not participating in a school sport: Please fill out the self-assessment form. Without the attestation form, they will not be able to participate in Physical Education. This form is a family’s attestation on the severity of their child's illness.  If "yes" is answered to any of the questions, physician clearance will be required for your child to resume activity.  Please reach out to your school nurse if have any questions or need further clarification on this.
    • The forms are available below.
