What is needed to register your child?

• Verification of Birth (certified copy of birth record, statement by the local registrar recorder certifying the date of birth, baptism certificate, passport, when none of the foregoing is obtainable, an affidavit of the parent, guardian,  or custodian, or any other appropriate means of proving the child's age) 
• Current Immunization 
• Polio (4 doses)
• DTP (5 doses)
• MMR (2 doses)
• Hep B (3 doses)
• Varicella (1 or 2 doses)
• Physical (Required for Kindergarten and First Grade)
• Proof of Residence (utility bill, rental lease, pay stub, or government-issued letters such as SSI, disability, and Social Services) that must be current within the last 30 days and must be under the parent's name.
