COMPLETION OF DUTY FORM

The Completion of Duty form has been converted to an online submission form in Google Drive. 

After fulfilling your contract, please click this link  Completion of Duty  and submit to the immediate supervisor.  (This may be your building principal or the athletic supervisor.)   

The title of your supplemental should match what is on the contract you signed (making up your own title slows the process down.)

You may submit name & address changes online using the Employee KIOSK.  Log on to your KIOSK account, select Profile, and then click on Request Profile Data Change.  Please submit new tax withholding forms to complete the process. (W-4 and OH-IT4)
