    ACADEMIC CLASS SCHEDULE CHANGE REQUEST FORM 

    The Academic Schedule Change Request Form will be open to 9th-12th grade students during Spring 2025 Add-Drop WEEK, 1/13/25 - 1/17/25.  ALL Applications for Academic Schedule Changes are due by Friday, 1/17/25.  
    El formulario de solicitud de cambio de horario académico estará abierto para los estudiantes de 9.º a 12.º grado durante la SEMANA de 13/1/25 al 17/1/25. TODAS las solicitudes de cambio de horario académico deben presentarse antes del viernes 17/1/25.

    ​This form is to submit any academic issues you are having with your academic classes (ELA, HP, Math, Science, language.)   Submitting the form does not guarantee the change will be made, but we will reach out to confirm the result via email on the date specified.

    How Requests Are Considered
    1. Students must submit a schedule change form during the Add/Drop Week in order to be considered for a change for the current semester.
    2. Applications are considered by staff.  If you submit an application, be sure to check your email daily in case you are invited to a meeting to discuss your application.
    • Switches are made for academic reasons.  If you are experiencing a social or behavioral issue with a peer, please inform a trusted staff member who can help you solve the problem with the support of the Admin team. 
    • We will be checking with your current teacher regarding your behavior and effort in the class.  If work completion or behavior is an issue, that will reduce the chances of a change being approved.  
    • Ms. Bryant reviews applications to make sure they meet the criteria.  Then, Ms. Hum makes final decisions regarding Advisory requests; Dr. Nguyen makes final decisions regarding Class requests.  You will receive final confirmation in email the week of Midterms.
    Tips for a Successful Application (read carefully)
     Read all instructions carefully.  Proofread your spelling, capitalization (no lowercase "i"!) and use full sentences.  Have a trusted staff member look over your application before submitting. 


    Este formulario es para enviar cualquier problema académico que tenga con sus clases académicas (ELA, HP, Matemáticas, Ciencias, Lengua). Enviar el formulario no garantiza que se realice el cambio, pero nos comunicaremos con usted para confirmar el resultado por correo electrónico en la fecha especificada.

    Cómo se consideran las solicitudes
    1. Los estudiantes deben enviar un formulario de cambio de horario durante la Semana de Agregar/Eliminar para que se los considere para un cambio para el semestre actual.
    2. El personal considera las solicitudes. Si envía una solicitud, asegúrese de revisar su correo electrónico todos los días en caso de que lo inviten a una reunión para analizar su solicitud.
    • Los cambios se realizan por razones académicas. Si tiene un problema social o de comportamiento con un compañero, infórmeselo a un miembro del personal de confianza que pueda ayudarlo a resolver el problema con el apoyo del equipo administrativo. 
    • Consultaremos con su maestro actual sobre su comportamiento y esfuerzo en la clase. Si la finalización del trabajo o el comportamiento son un problema, eso reducirá las posibilidades de que se apruebe un cambio.
    • Ms. Bryant revisa las solicitudes para asegurarse de que cumplan con los criterios. Luego, Ms. Hum toma las decisiones finales con respecto a las solicitudes de Advisory; el Dr. Nguyen toma las decisiones finales con respecto a las solicitudes de clases.
