Emergency Student Information Form

OCPS requires a new emergency student information form for all students every school year. Please complete the form and return it to the school as soon as possible. Please list multiple adults who can be contacted in case of an emergency. If an early pick-up is needed, only those individuals listed on the form will be permitted to pick up your child from school. Medical Notes

If your child is absent from school and you have a medical excuse note, please give it to the attendance clerk. If your child has a doctor’s note with special instructions (e.g., no gym/sports, must wear brace, allowed to use crutches, etc.), please give it to the school nurse and she will notify the teachers.

Sports Physicals

Students who want to play on a school-sponsored sports team (soccer, volleyball, basketball, or track) must have an up-to-date sports physical on file. Sports physicals are valid for one year. Sports Physical Form.

Immunizations

OCPS requires that all students be up-to-date with their immunizations. Students entering 7th grade may need a Tdap booster, which must be recorded on an updated DH680 form from the physician. Please talk to your health care provider about whether your child is up-to-date with immunizations.

Medication at School

The school nurse can administer prescription and over-the-counter medications during the school day, if needed. The parent/guardian must bring the medication to the clinic in the original pharmacy bottle with the label attached. If you would like to keep over-the-counter medication in the clinic (e.g., cortisone cream, benedryl, eye drops etc.), the parent must bring in an unopened bottle. Students may not bring medication to school or carry it in their backpacks except for Tylenol/Advil, emergency medications such as inhaler, EpiPen, and diabetic medication with self-carry form. Please complete the medication packet form below and bring it to the clinic along with the medication.

**Please note: In accordance with OCPS policy, if students are found with medication or unauthorized inhalers, EpiPens, etc., the items will be taken and the parent/guardian will need to come to the school to pick up the items.

Students with Asthma and Inhalers

If your child has asthma, you can store an inhaler in the clinic. If you would like your child to self-carry an inhaler in his/her backpack, you must complete the Self-carry form and have it signed by your family’s health care provider. Students are not permitted to self-carry an inhaler without a completed form.

Students with Severe Allergies and EpiPens

If your child has severe allergies and requires an epinephrine injector (e.g., EpiPen, Auvi-Q), you can store it in the clinic. Students may also self-carry an injector in their backpack as long as the appropriate paperwork is completed. Several forms must be completed by the parent/guardian and the health care provider. Please make an appointment with your health care provider to have the forms completed and signed as soon as possible.

Students with Diabetes and Diabetes Testing Supplies

If your child is diabetic and needs to self-carry testing supplies and insulin, several forms are required from the health care provider. Please make an appointment with the health care provider to obtain a current copy of the Diabetes Medical Management Plan and for the appropriate signatures on the Self-carry form.
