Verification of Residency

Documents Required to Verify Residency

All families are required to provide the following documents to verify residency (or to re-verify residency after a move within Mill Valley):

1. Declaration of Residency

2. ONE of the following documents showing the name and address of the parent, guardian, or caregiver within the District:

• Copy of the deed
• Copy of the most recent mortgage statement
• A fully executed copy of the lease or rental agreement, PLUS Lessor/Lessee Form completed by Landlord

3. ONE of the following documents showing the name and address of the parent, guardian, or caregiver within the District. Must be dated within thirty (30) days of school registration documents.

• PG&E
• Marin Municipal Water District
• Mill Valley Refuse/Tamalpais Community Services District/or other local refuse agency serving residences within district boundaries
• Comcast (cable TV)/Direct TV
• Landline telephone (cell and long-distance service not accepted)

4. TWO of the following documents showing the name and address of the parent, guardian, or caregiver within the District.

• Valid California Driver's License
• Current Vehicle Registration card
• W-2 form dated within the past year
• Current pay stub
• Property tax bill/payment receipts dated within the past year
• Correspondence from a government agency or Public Assistance documents
• Bank or credit card statement dated within the past forty-five (45) days
• Dental or medical bill dated within the past forty-five (45) days
