As outlined in the LPS Code of Conduct, harassment and bullying are serious offenses and will not be tolerated.

To report alleged harassment and bullying that occurred on school property; at a school-sponsored activity or event off school property; or on a school bus, please download and complete this NJDOE Form 338. Please note, this form has limited characters under Incident Information.  If there are more than 200 characters, please attach a separate document.

If you are a student victim, the parent of a student victim, or a close adult relative of a student victim, and wish to report an incident of alleged harassment or bullying, COMPLETE THIS FORM - NJDOE Form 338. Please note, this form has limited characters under Incident Information.  If there are more than 200 characters, please attach a separate document. 

Once Form 338 is completed, please return it to the Principal at the student victim’s school. Contact the school for additional information or assistance at any time.

1. a reasonable person should know, under the circumstances, will have the effect of physically or emotionally harming a student or damaging the student's property, or placing a student in reasonable fear of physical or emotional harm to his person or damage to his property;
2. has the effect of insulting or demeaning any student or group of students; or
3. creates a hostile educational environment for the student by interfering with a student's education or by severely or pervasively causing physical or emotional harm to the student.
