Student Health
School nurses ensure that students and staff are able to learn and work in a safe and healthy environment. Kuna School District's school nurses are dedicated to providing the following health services:
- Emergency and urgent situation management
- Assessment of health complaints and care for students with special health needs
- Identification and management of students' chronic health care needs that affect educational achievement
- Diabetes management
- Medication administration
- Hearing, vision, and scoliosis screening
- Health, growth and development education
- Immunization management
- Function as leaders of the school health services team that includes clerical staff, classroom aides, school counselors, school psychologists, and school social workers
IMMUNIZATIONS FOR SCHOOL
Children must meet immunization requirements as stated in Idaho law to attend school. To meet legal requirements, a record with evidence that children are up-to-date on their immunizations (shots) or a valid immunization exemption must be provided and kept on file at the school.
You will need to present your child’s immunization record or a valid immunization exemption to the school at the time of registration to enroll your child.

Exemptions
Idaho law allows a parent or guardian to claim an immunization exemption for their child for medical, religious or other reasons.
- A medical exemption must be completed by a licensed physician.
- It is recommended that exemptions for religious or other reasons be documented on the form provided by the Idaho Department of Health and Welfare, Immunization Program.
- Parents or guardians may also claim an immunization exemption by providing a signed written statement when entering their child into school and/or childcare.
In the event of a disease outbreak, children who have claimed an exemption and have not received the immunization against that disease may be excluded from school and/or childcare.

The following questionnaire forms are required for students with the these conditions:
Medication
If a student must take prescription or non-prescription medication during school hours, a "Medication Consent Form" must be signed and returned to the school by a parent or guardian. The medication should be in the original container (your pharmacy will usually make up the prescription in two separate containers) and the student's name, prescription number, doctor, and directions must be clearly written on the container.

Athletics
All students participating in interscholastic athletics must complete the Idaho Health Examination and Consent Form.
