All school volunteers must complete this application form in order to volunteer in the Clovis Unified School District ("District"). For your safety, and that of the District's students, a background check will be completed on all applicants. 

*NEW* CUSD is using the RAPTOR system for all visitors and volunteers on our campus (or chaperoning our kids on trips).  The volunteer form is now ONLINE.  Link providing at the bottom of this page. All volunteers must fill this form out ONLINE and attach proof of TB assessment OR TB Test.  With no TB assessment/test form, volunteers will not be allowed.  

To the right of this page is the Volunteer TB Risk Assessment Clinic Schedule.  Parents can go to the clinic on the scheduled dates and be assessed.  If they don’t need a TB test, the Nurse will sign off on a form they can either upload with their application OR bring it to the main office.  If they DO need a TB test, they will have to go to their health care provider to have the test, and upload that document or bring it to the main office.

**If they volunteer at an elementary school and have turned in their TB paperwork already, all information will work at all school sites.
