The following information is required to register your child at Clovis West High School:

1. Parent or Legal Guardian must be present

2. Proof of Residency (Must be in Parent’s/Guardian’s name)-One of the following:

   TYPE	DATED
   Property Tax Payment Receipts	Less than 6 months old
   Lease of Rental Property	Valid on the date of presentation
   Rent Receipts	Less than a month old
   PG&E Bill or Bill/Receipt	Less than a month old
   Water, Sewage and Garbage Bill/Receipt	Less than two months old
   Pay Stub	Less than a month old
   Correspondence from Government Agency	Less than two months old
   Voter Registration	No date restrictions
   Escrow Papers - Pre-owed Home	Less than 45 days until closing
   Purchase Agreements - New Home	Not a valid proof of residency - this document is verification to be submitted with transfer applications only
   Closing Papers - New Home	Less than three months old
   Correspondence from a Government Agency	Social Security Payments Welfare Checks, Welfare Eligibility Documents Department of Social Services Correspondence IRS Tax Refund Checks

3. Current Immunization information: 

   DTP/DtaP/DT/Tdap - 5 doses

   Tdap - 1 dose

   POLIO (OPV or IPV) - 4 doses

   HEPATITIS B - 3 doses

   MEASLES, MUMPS, RUBELLA - 2 doses

   VARICELLA - 2 doses

4. Certified Birth Certificate

5. Unofficial Transcript (Withdrawal Slip with drop grades if registering during the school year)

6. Parent/legal guardian photo ID
